Prospective Volunteers
Dear Prospective Volunteer,
Thank you for your interest in the volunteer program at Phelps Memorial Hospital Center.
At the core of this program is a group of more than 200 committed and enthusiastic volunteers who play an important role in fulfilling the Hospital’s mission and values.
You
may fill out the volunteer online application form
or print and complete the form and mail it to: Volunteer Office, Phelps
Memorial Hospital Center, 701 North Broadway, Sleepy Hollow, NY 10591. You are
welcome to email pcardozo@pmhc.us if you
have any questions. Once we receive your application, we will call to schedule
a personal interview during which we will discuss current volunteer opportunities,
your interests and availability.
In addition to the application, you will be asked to complete a medical clearance/confidentiality form. A simple test for tuberculosis is also required, for which there is no charge. Phelps will provide you with a uniform at no cost, and training in your specific volunteer service area will be given.
We
